The man forfeits his parental rights when he has sex with a woman, not his wife. He must hire an attorney to petition the courts to reestablish his rights. She has sole custody and control. It's a glass ceiling thing, only the men are relegated to the subbasement with no stairs to climb.

The mother usually gets residential custody and the father would have access.
References :
Usually the mother gets full custody and the father gets access rights but there are always exceptions. If the father can demonstrate she is an unfit mother or if the children are old enough for their wishes to be taken into consideration (depending on which country you live in) and they wish to live with their father, then she may not get full or even joint custody.
References :
Well that depends on the behaviour of the mother towards her kids, if she mistreats her kids, is a drug addict, or an alcoholic, then she isn't fit to look after herself let alone her kids, and so the court would award custody to their next of kin, which would be their dad that if he's in their life, if not would then be a grand parent or a family member
References :
That's what usually happens, what should happen is that the better parent gets custody by the courts are skewed in favour of the mother.
