How does a parent give custody or guardianship of a child to someone?

My mother wants to give me guardianship or custody of my little sister but we don’t know how to do it. She lives out of state and is having trouble getting someone to help her, because they all tell her they don’t have jurisdiction in the county that we live in. How can we do that? Ans who do we contact?
